• the completed “application for admission” form
• a copy of the student’s birth certificate
• an I.D. photo of the applicant
• a copy of the last school year’s report
• a written recommendation by the applicant’s teacher
• a letter from parents explaining why they are interested in our school
• a letter of recommandation for the family and student
